submit it to us by pressing the "Submit" button when you are
finished. If you need to submit a Reservation Request for more items
than this form allows please complete as many requests as needed for
your event.
Use the drop down boxes to select your
equipment and then specify quantity and color where appropriate.
Use the memo box at the end to let us know about any
special equipment requests or questions that you might have.
Use of this form does not guarantee that the
equipment you are requesting will be available for the date/time you
have selected.

representative of Prime Time Party Rental will contact you to assist you
in finalizing and confirming your reservation as quickly as possible.
